Digital Health Card for every Telangana citizen; Sangareddy chosen for pilot project

Sangareddy: The district of Sangareddy has been selected as a pilot project for the Telangana Digital Health Profile project, said Health Minister Damodar Raja Narasimha on Thursday.

The initiative aims to create a comprehensive digital medical history for every citizen, streamlining treatment from primary health centres (PHCs) to super-speciality hospitals.

64 facilities identified

The announcement came following a coordination meeting chaired by the minister at the Sangareddy District Collectorate. The officials presented a technical blueprint of the platform, detailing how patient data will be integrated across various levels of the state’s healthcare hierarchy.

The pilot project will be rolled out across 64 government healthcare institutions in the district, creating a dense network of digital touchpoints:

One Government General Hospital (GGH)

4 Area Hospitals and 5 Community Health Centres (CHCs)

4 Urban Primary Health Centres (UPHCs)

31 Primary Health Centres (PHCs)

19 Basti Dawakhanas

How will the Digital Card work?

Under this system, every patient visiting a government facility will be issued a Digital Health Card. This card will act as a key to their digitised medical history, recording every step of their clinical journey:

Registration: Real-time logging at the Outpatient (OP) desk.

Consultation: Digital entry of symptoms and diagnosis by Medical Officers.

Diagnostics: Direct integration of lab investigation reports.

Pharmacy: Digital prescriptions to track medication history.

How will the Health Card benefit public health?

Minister Raja Narasimha emphasised that the primary goal is to move from reactive care to proactive management.

Immediate Clinical Decisions: Doctors can access past records instantly, reducing the risk of redundant tests and ensuring faster diagnosis.

Specialised Care: The profile will specifically track Non-Communicable Diseases (NCDs), maternal and child health (MCH), and infectious diseases.

Seamless Referrals: The system is designed to trigger early warnings for critical conditions like cancer or cardiac issues, allowing for a smooth, automated referral to super-speciality hospitals.

‘Sangareddy must become a model for Telangana’

The Minister has directed the District Collector to expedite the installation of high-speed connectivity and necessary IT infrastructure at all identified centres.

“Sangareddy must serve as a model for the entire state,” the Minister remarked, noting that the success of this pilot will dictate the framework for a state-wide rollout.
